ריטאלין - איך זה עובד!

לאור ריבוי השאלות בנושא הריטאלין לאחרונה בפורום, להלן סיקור על מנגנון הפעולה:
קריאה מהנה.

חדשות לבקרים עולות טענות שונות, מרביתן חסרות בסיס, בגנות השימוש בריטאלין. התרופה מוכרת זמן רב כיעילה ביותר לטיפול בהפרעות קשב. אחד הדברים שלא היו ברורים עד כה הואכיצד תרופה מעוררת משפיעה כל כך טוב דוקא על ילדים היפראקטיביים? התרופה כלולה במסגרת הסמים, על פי פקודת הרוקחות, אך מסתבר שאינה ממכרת כאשר נתנת ונילקחת על פי ההתוויות הרפואיות הראויות.

מאמר שפורסם השבוע, שופך אור על מנגנון פעולת הריטאלין ונותן תשובות לתהיות רבות שהיו קשורות בשימוש בתרופה. מסתבר שמנגנון הפעולה של הריטאלין נעשה ע"י השפעתו על רמת הדופאמין (Dopamine). הדופאמין הינו חומר כימי המשפיע על תחושת ההנאה וגורם להגברת ההנעה (מוטיבציה). ילדים עם הפרעות קשב סובלים מקשיי ריכוז והתעניינות ולכן גם לא מבצעים את המטלות שלהם למרות קיום הפוטנציאל הבסיסי.

ההנחה היתה שהריטאלין (או בשמו הגנרי מתילפנידט) מעלה את רמות הדופאמין החוץ תאיות. עבודה מוקדמת בחיות, לימדה שהזרקת ריטאלין מביאה לחסימת הטרנספורטרים - האחראיים להכנסת הריטאלין לתאים, וע"י כך עולה רמת הדופאמין מחוץ לתאים ומושג האפקט הרצוי. המינון שניתן בניסוי זה היה גבוה מהמקובל ולכן נשארה התהיה האומנם כך פועל הריטאלין במינונים הטיפוליים.

העבודה הנסקרת, בחנה ב-11 מבוגרים בריאים שאינם צורכים תרופות כלשהן, באמצעות (Positron Emission Tomography (PET, ע"י הזרקת חומר מסומן המתחרה עם הדופאמין על אותם נשאים וטרנספורטרים. במקרה זה אם אכן הפעולה היא כפי שנמצא בחיות, העיכוב התחרותי של הדופאמין, ימנע כניסתו לתאים ויעלה את רמתו מחוצה להם. בדיקות ההדמיה נעשו לפני ואחרי מתן ריטאלין.

מינון הריטאלין שניתן היה 0.8 מ"ג לק"ג משקל גוף. נמצא, שמתן הריטאלין העלה רמת הדופאמין החוץ תאית. מימצא זה מצטרף למימצא נוסף לפיו יש בחולים עם הפרעות קשב ריבוי טרנספורטרים לדופאמין ולכן סילוק החומר ופרוקו מהירים יותר בחולים ומונעים את ההשפעה הרצויה. הריטאלין החוסם אותם כאמור מעלה את רמת הדופאמין ומאפשר לילדים החולים להגיע לרמות הריכוז הנדרשות כדי לבצע אותן מטלות שקודם לא יכלו.

הריטאלין אינה התרופה היחידה המעלה רמות דופאמין. מדוע אם כן אלכוהול או קוקאין או אמפטאמינים אחרים מקבוצת הריטאלין אינם פועלים באותו אופן? מדוע הם ממכרים וריטאלין אינו ממכר?

התשובה קשורה במהירות הספיגה והחדירה למוח. משך הזמן שבו נספג הריטאלין ארוך משמעותית מהאחרים ולכן אינו מעורר תחושת "high" במשתמשים. יחד עם זאת יש להזהיר שבמצבי שימוש לרעה של התרופה, שבה מסניפים אותה כאבקה או מזריקים תוך ורידית ניתן לגרום למצבי התמכרות.
